CRITERIA FOR NUTRITIONAL PATIENTS NEEDING EVALUATION
1. Albumin level < 3.5
2. 10% weight loss in the past 3 months
3. Malnutrition diagnosis or difficulty with eating, chewing, or toleration of food
4. Slow or no change in wound measurements
5. Gastrointestinal problems i.e. malabsorption, non-absorption, gastro-paeresis, ulcerative colitis, diarrhea, nausea, and vomiting
6. Failed enteral feeding
7. Nutritional levels are not better even with oral supplements and or enteral feedings
8. Other conditions – for patients who are renal dialysis, hx of cancer or receiving cancer treatments, or inability to eat for whatever reason should be screened for malnutrition and/or malabsorption
9. Patients on Negative Pressure Wound Therapy
